• Name of the person(s) whom you are honoring.
• Home address or email of the person being honored. If the person is deceased, please include contact information for a loved one.
• Reason for the tribute (in honor or in memory)
• Any message you would like to accompany the acknowledgement.

We will send an acknowledgement of your gift (amount not included) to the person being honored. If you prefer to deliver the tribute card in person as a gift, please let us know. We are happy to work with you to make your gift giving special and meaningful.

Recent tributes are also listed in the back of our upcoming newsletter. For acknowledgment in the post-holiday newsletter, gifts must be received by January 31st.
